MEDICAL ALERTS: ANAPHYLAXIS & ASTHMA

Please update your child's medical information in our files! 

If your child carries an epi-pen (anaphylaxis) or an asthma inhaler (asthma), please submit updated forms to the office, along with the registration. 

  • Bring new medication (epi-pen or asthma inhaler) to the school in September

  • Current medication will be returned at the end of the school year
